For patients using our NHS Prescription and repeat prescription service we will need to know whether you pay for your NHS prescriptions and if not the reason that you are exempt from payment (akin to the back of the traditional green prescription form). We also collect information about your existing medical conditions and any other prescription medication that have been or currently are prescribed for you by your doctor. Our systems will store the details of all medicines and products that we have supplied to you on either an acute or repeat basis.
